A designated van exclusively services Bremen on Mondays, Culver on Wednesdays, & Bourbon on Thursdays

$ 2.50 per boarding, within 5 miles

5 - 30 miles - $6 one-way; 31+ miles - $11 one-way

FREQUENT RIDER DISCOUNT CARDS ARE NOW AVAILABLE!

If you are 60 or over, Title III Older Americans Act funds are available for your ride. 

Medicaid transportation is also provided.

8:00 am - 3:30 pm, Monday through Friday. 

The intake coordinator will take your personal and appointment information, including any special needs (w/c equipped van, oxygen equipment, attendant, etc) and the scheduler will return your call when scheduled.  Please have your Medicaid number available as well, if applicable.

 

Special Services

Wheel chair accessible vehicles available at no additional cost.

Personal attendants, service animals, and / or Oxygen equipment ride free of charge.

Materials are available in alternate formats upon request or by calling Relay Indiana 1-800-743-3333

To assist us with on-time service, please be ready 15 minutes before your scheduled pick-up time.
